Our primary location is located in the beautiful city of Upland, CA.  We are walking distance from downtown Upland, as well as the Upland Metrolink Station and serviced the Rout 83 Omnitran bus.

***Event Parking: Please be aware that the parking lot in back of the office fills quickly, so please park on  Euclid Ave (southbound side).

 Other Locations: We are currently seeing patients by appointment only at our Sherman Oaks location in the Triad Wellness Center office of Dr. Jorege Arvizu and Dr. Martin Stites.  We are only at this location Fridays & select Saturdays.  Please call for more details (909) 949-0155.
